Lost Ticket Indemnity

Complete the Lost Ticket Indemnity (LTI) to request the reissue or refund of lost or stolen documents.

Documents or coupons include the following:

  • Air tickets
  • Miscellaneous Charges Orders (MCO)
  • Multi-Purpose Documents (MPD)
  • Any of the above that have been defaced

Actioning the LTI

Once you have completed the online LTI form, you will need to:

  • Print 3 copies (airline, office, customer copy).
  • LTI form signed by the customer and witnessed (2 Signatures, customer and witness).
  • Attach a copy of the original ticket.
  • Passenger must be given a copy of the refund form and ticket.

Air New Zealand assisting with the LTI reissue

For a customer who has lost their paper ticket during their travel and Air New Zealand is to reissue the ticket on your behalf, all ticketing details are to be entered as OSI's.
To re-issue a replacement document the following information is to be obtained:

  • Ticket Number
  • Issuing Office or Agent Name and IATA Code
  • Date of Issue Flight Details/Routing
  • Country of issue
  • Fare Calculation/ROE (rate of exchange)
  • Fare Basis Code
  • Tour Code (if applicable)
  • Endorsement/Restrictions
  • Currency of Issue
  • Fare
  • Taxes
  • Total Fare
  • Original FOP Details

Air New Zealand charges a service fee of $75.00 per person, per ticket or the local equivalent fee where applicable for lost/stolen (LTI) to be processed.

Advise your Customer(s) to allow extra time to complete lost ticket formalities prior to check-in if reissue is at the airport.
